The Safari browser has crashed. How can I get it going again?

i have an alert on the ipad which reads, (are you sure you want to submit this form again? cancel - submit) and i can't get rid of it

Close out the app and then remove it from the recently used list. Go to Settings - Safari and then clear the cache and history. Restart the Safari browser.
