    there are some odd shaped spots that look like rips on the screen and they are dead spots. They are only there during guided access.

    Not sure. But guided access is designed so that you are able to specify areas of the screen that don't respond to touch. For instance if you wanted to make sure a button couldn't be pressed. Maybe you are accidentally setting this up? Or there's a bug in it.
