Why doesn't my Macbook Pro autofill work for passwords not name, address and phone?

my macbook pro autofill works for passwords not name, address and phone

Go to Safari > preferences > autofill and make sure that all the box that says 'using info from my contact card' is checked. You may also need to click on the edit button and make sure all your address info and what not is filled out.
