iTunes Form Creator

Hello everyone,

I was wondering if anyone knew of an app on iTunes that allows me to create my own forms with drop down boxes, where I can select certain text that has already been saved(previously). I would need to create my own form, since it would be custom to only my business. I would also need it to be able to be exported to say a PDF or word document format, so I could print it. If anyone knows of one or of a free trial edition where I could try it, please let me know, that would be great. Thanks in advance

Also you might want to look at Genius Scan which is free and can be found in iTunes. It is highly customizable and allows the built in camera to be used to shoot documents and turn them into PDF format and do many things with them.
