I am in the US, a friend in Italy. I know WiFi will work FREE OF CHARGE on WiFi. But if my friend does not have WiFi in certain areas, can she use FT anyway and just be charged cellular data fees?

That is my understanding, but she should double-check with her carrier, just in case.

That is what I originally thought, just would be some hefty data fees if not on WiFi. Didnt know if she would need to dial the country's code first, even though I am a contact in phone and she can simply hit FT.

Just her normal data fees. Should be no need to dial country code.
