How to get facet time icon on my ipad5?

Do you mean the iPhone 5? The iPad 5 isn't even out yet. If you're missing an icon sometimes rebooting your iDevice will bring them back.

Also try using Siri "launch Facetime" or doing a search by pulling down anywhere on the screen except from the top or bottom which will bring up Notification Center or Control Center respectively. Type Facetime in the search bar and you should see the icon. Sometimes the icons are burried on another home screen or it may be in the Utilities or another folder.
