A guy at work has a work 4 and his is bent from changing cases.

I got one of these from someone who seems like a reputable dealer who claimed that it came bent from the factory but Apple refused to replace it for them.
It was bent around the volume button and the glass on the top was coming out of the frame.

I brought it to the Apple store in its box with the plastic screens still covering the phone and Apple replaced it for me. I did not have it activated at the time and there were zero calls from the previous owner who merely said he activated it and brought it straight to Apple. There were no signs of use on the phone (the screen was clear). The phone was new by the time I exchanged it (about 3 weeks old).
