It's not a program. It's connections inside apple and at&t.

several places on the internet and ebay offer this service, it goes from 10-20 dollars, sometimes cheaper, as far as I know it can't be done by yourself

The price depends solely on the carrier. For Vodafone UK the cheapest price I could find was around $120 - mind you, that was the cheapest. Most hovered around $200.
