Unlock 3G still not working...help

I'm pretty frustrated at this point....I downloaded the iPad base band from ( Dev-Team Blog). Then I ran the Redsn0w program again (4th time) from the iClarified website. In the step by step Redsn0w process it never shows or gives the options shown in the screen shots below which are in the instructions?? It bypasses them every time to the button holding step. Any suggestions?? What am I doing wrong?? What's going on?? When I did this last night, my phone is now jailbroken with cydia on it. How do I unlock, untethered for T-Mobile? 3G 4.2.1 & 05.15.04
