When I got my first iPhone a few days ago, I noticed I had a lot of trouble with hearing someone on the earpiece and being able to hear the ring, NOT ANYMORE. 
Off of Cydia I downloaded Carrbone's Speaker, Volume Boost and off the internet I searched for LOUD iphone ringtones, found a website and downloaded directly to iTunes, phasar-to-stun.
I can hear callers GREAT now and the ringtone is LOUD and I can easily hear it!
I am REALLY pleased with these apps.
