scan into a web form

I am looking for an application that we allow us to scan tickets into a web form. We are going to use Ip phones and the form has to allow us to set the start url, form field, etc. This will be used to check that the ticket is valid and not been used. We already have a application that creates tickets with barcodes. The application can be a paid application.
