iphone 6+ automatically changed passcode??

My wife's iphone has 6 digit passcode, but this morning when she turned on her phone, the passcode changed to 4 digit. She tried her old passcode, which she used when it was 4 digit, but it didn't work.

What happened?? She's 100% sure she didn't change the passcode.

Did she do an update by any chance or reinstall the software? It sounds like it went back to the default selection, but this only happens if new software has been installed. Can you provide a bit more information on what happened?
