Welcome to iMore! Try going to Settings > General > Accessibility > Toggle LED alerts off. > do a hard reset:
Hold the top or side power/sleep button and the home button at the same time until the Apple logo appears, then release. >
Then toggle LED alerts back on.
Let us know if this works or not.

This is probably a remote possibility, but the phone will need to be in a position that does not block the flash. If the phone is on a table or other item that doesn't allow light to escape, then it could be possible that you won't be able to see the flash.
