I've used assistive touch almost from the beginning of my iPhone 5 purchase as well as a LP case which covers the mute slider switch on the side of the phone
I always had what I assumed was a Mute Bug that carried over since iOS 6.2 when I reboot my phone it would Mute all sound on my phone until I went to the mute icon on assistive touch and tap the mute sound back on
Low and behold after finally taking my LP case off I realized I had the mute switch on Mute the whole time it seems to work like a reverse light switch when your using the assistive touch accessibility app
Now I'm thinking this also had something to do with losing sound notifications on a few instances I will start to monitor any other changes for on
