iPhone 4 - Activation Error - Please Help

Hello all I got a iPhone 4 White to repair that was restored and I cannot seem to activate it, i get the message "Acvtivation Error , Activation request could not be completed. If the problem persists, please contact customer care." on the phone itself. Tried several restores and all I get from the computer after the restore/update is... "We're sorry, we are unable to continue with your activation at this time. Please try again later, or contact customer care."
Got a full checkmend report and the phone came back clean.
Anyone see this problem before? Is the phone broken? Please help.

sounds like it might be on the "lost or stolen" list- gotta call the carrier

Checkmend report shows not stolen or lost, came across other things saying that it might be previously jailbroken and may never work?...

I'm not sure. I would call the carrier.
