Bluetooth 4.0 and iOS 5 Confusion

Prelude: I have an iPhone 4, got it back when they came out and it's running iOS 5.0.1.

Bluetooth is a pretty handy feature to have on the iPhone yet I usually keep it off to save battery. Keeping it on left that little symbol in the status bar, letting me know my battery was being drained slightly faster. My dad got an iPhone 4S recently, and one thing I noticed that if he keeps his Bluetooth on, the symbol disappears from the status bar when he leaves the Bluetooth section of the Settings app.

I originally figured this was something with Bluetooth 4.0, but my grandma got the 8GB iPhone 4 for Christmas, and her iPhone does the same thing.

So my question is this: Does the lack of the symbol mean its actually not active? Does it save battery? Is it related to Bluetooth 4.0? (I'd think not because an iPhone 4 does it.) Finally, is it some setting that I can turn on for my iPhone?
