Hide My Phone Number Feature

To those of you with iOS 5.1.1, do me a favor and go to Settings/Phone and see whether or not the call forwarding, call waiting and Show my phone number features are present. Mine are no longer present and I don't know whether or not it has been that way since I updated to 5.1.1 or whether my device needs to be reset as new. Any assistance is appreciated. I've attached a screenshot of what my phone settings look like, by the way.

Thank you for the speedy reply. I appreciate it.....Unless it's a Verizon thing, those feature should also still be available to me. Me & my spouse both have an iPhone 4S via Verizon and those features are missing on both of our devices. Interesting. Anyway, thank you again.
