The loudspeaker turns on seemingly without any action from me.

Assuming in would be an iPhone on the call screen there are 6 icons and top far left is your spkrphone which is pretty hard to randomly have turn on by itself so if you have it on just tap that icon off.

On my iPhone 6S the speakerphone is on the top right.
