How can my playlists start playing over my wifes Iphone (over my car stereo).

I left my phone at home, she had hers and the stereo just started playing songs from her phone. Even if I inadvertently had my music shared, it certainly wasn't shared specifically to her. Weird, weird, weird.

So, your wife's iPhone began playing your playlists through the car stereo? Was it music you purchased through iTunes/Apple Music?

Are you sure you don't have have Apple Family Sharing enabled?
