Email question

Not really an Apple question but a general question about emails.
I've noticed a few times emails from people that seem to have an email address with their last name. Example: if a contact is John Smith, they have an email address such as: John@smith.com. They don't have a website or business but they have an email address with their family name.
How does one get this? Is it expensive?

I did this with one of my premium Outlook.com accounts when it launched.

The challenge is to find a variation of your desired last name that's available.
