How do you go about getting a refund from iTunes purchases?

My mom has been receiving payment from Itunes on her debit card and wants them taken off right away.

How does she go about getting a refund for these purchases. She did not purchase them and would like the money put back into her account.

She needs to contact her bank immediately to file a fraud claim and have a new card issued with a new number. The phone number will be on the back of the card.
