How do i make a blank page to be the default page on my iphone 5s ios9.1?

I used to be able to use a blank page as the default page on my phone instead of the favorites page. I accidentally deleted the blank page icon and when I put it back it is just part of the default favorite page.

Did you used to launch the Safari app or the blank page you created? If the latter, re-create the blank page entering about:blank in the address bar of Safari. Afterwards, tap the share button and then select "Add to Home Screen".
