Remote Hotspot Failure

The Instant Hotspot (or whatever it is called) has been non functional for me. I keep getting g the remote hotspot failure message on my iPad. I ended up resetting network settings on my iPad and iPhone 6. Now this is what I see. In settings under remote hotspot I see "iPhone (4)". I also see "iPhone" under the list of wifi hotspots. I have to click iPhone and enter my wifi password but the personal hotspot "iphone (4)" is still there

Any idea on how to fix this or at least get rid of the personal hotspot connection since it's useless?

I ended up resetting all settings. The Instant Hotspot no longer shows up, I can connect to it the "old fashion way" just fine. It would be nice if apple had a fix for this.
