Where do you place your ear when you talking on the iPhone 4?

My spouse is sometimes having difficulty hearing on her iphone 4 even though she has normal hearing.

Has she tried increasing the volume using the side volume buttons while she's talking on the iPhone?

As for where to place her ear, well, have her place her ear in the same manner of a typical house phone - near the top of the device.
