How do I get my contact icon back?

Unless you are jailbroken it's impossible to hide or delete the icon. If it's not visible on your screen, it must be in a folder. Use Spotlight to find it - pull down on a blank space on any home page and type contacts in the search box.

And IF it is in a folder, the name of the folder will be listed in the search results screen to the right of the app name ("Contacts"), so you can find it and move it out of the folder if that's your desire.
