Why do I get different call screens when receiving phone calls?

somtime i get call screen shown on left side sometime i get call screen on right side ..... why ? and it is also for the same contact too

The screens are different because of the state of the phone. In the left photo, the screen is locked. In the right photo, it is not. Regarding the different names, is it possible that you have two entries in your Contacts with the same phone number?
