iPhone 4 would not be 4G and is this just something for Verizon. I have not used it in so long on Verizon I don't remember.

If you're using it to activate a Verizon phone, you'll need to be on Verizon's network because it won't program properly if you're not

With Verizon: *228 Option 1 - Activation, *228 Option 2 - PRL Updating. If you have a 4G LTE phone this will not work for Option 2. For Activation, I have never tried it with the 5 series. Also the phone has to be CDMA. What is the model number for your iPhone 4?
